When Do Wisdom Teeth Need to Come Out?

Impacted wisdom teeth: When a wisdom tooth can't fully erupt because it's blocked by other teeth, gum tissue, or bone. Impaction can cause pain, infection, cyst formation, and damage to neighboring teeth.

Crowding: Even when wisdom teeth erupt fully, they often push forward on adjacent teeth, causing crowding — especially problematic for patients who've had orthodontic work.

Recurrent infection (pericoronitis): The gum tissue around a partially erupted wisdom tooth traps bacteria and food, leading to painful flare-ups that keep returning no matter how well you clean the area.

Not every wisdom tooth needs to come out. If yours are fully erupted, properly aligned, and cleanable, we'll leave them alone.

What to Expect During and After Removal

Most wisdom tooth procedures take 30-60 minutes total. Simple extractions (fully erupted teeth) involve local anesthesia and feel similar to any routine extraction. Surgical extractions (impacted or partially erupted teeth) require a small incision in the gum and sometimes removal of a small amount of bone; these are typically done under IV sedation or oral sedation for comfort.

Recovery usually takes 3-7 days. Expect some swelling and bruising that peaks around day 2-3 and improves from there. We'll send you home with clear aftercare instructions, a cold pack, gauze, and prescriptions for pain management (often just ibuprofen is enough for simple extractions). Most patients return to work or school within 2-3 days; strenuous exercise and foods requiring hard chewing should wait about a week.
